Book excerpt: In order to gain access to gender-confirming hormones or surgery, transgender people must obtain a letter of authorization from one, or sometimes two, mental health professionals. This process, known as gatekeeping, is controversial among transgender people and their advocates, some of whom believe that transgender people should have the right to alter their bodies without needing legitimization from an outside source. Despite the controversy, there is very little empirical research examining the effects of gatekeeping on clients. The following is a review of the literature regarding gatekeeping and, also, a guide for practitioners who must act as gatekeepers for transgender people who are seeking access to medical transition.
